Does anyone have a copy (blank) of a form they have used, that a Vet would complete to state all teeth are present and bite is correct? Much harder to find than I had anticipated!! And yes I have called several reproduction Vets and none of them provide this paperwork and only one of the several I called actually understood what I was asking for!
