This really suggest to me to seek specialist opinion - my boy was really itchy with demodex which is again unusual and was actually the start of him developing atopy. Yes, my dog is itchy when his atopic dermatitis flares, but the the random Demodex bald patches he gets don't seem to be itchy/ painful at all. You might do better with a referral to a skin specialist. Demodex mites don't always show up on a skin scrape either. Crafty little buggers.
