They need the following information: 1. Was the tool called the 22 point checklist used as the method of identification? 2. Was the tool, (22 point checklist) used by a Queensland Animal Control Officer? 3.Which Council? 4.The name of the Animal Control Officer. 5.Do you have a copy of the Tool (22 point checklist) supporting documentation from the respective Council? If not, don’t worry as they can be obtained very simply. 6.All copies of correspondence between yourself and the respective council’s preexisting relationship. 7.If your dog was removed out of the state, or destroyed. 8.What dog breed/cross had you asserted it to be? 9.Your contact address/email and phone number.
